Outside of the normal consent stuff you may also wish to include whether or not information is secure (without https, it definitely isn't) so include warnings not to put anything private in the text fields.

Submitting Data
Well, not really, github doesn't allow server side code, so for this demo, you can simulate success or failure.
If you want to see your data, I recommend simulating failure 3 times. This will take you to a page which provides the users with their data and an email address to send it to (this is a real page in the experiment to provide a backup means of getting data if the server goes down mid experiment). The data will be in the text box. Be aware, however that this data is not formated in a way you can expect in a typical experiment. Successful transmission generates CSVs for each participant which can easily be merged into a single master spreadsheet.
After viewing your data, simulate success to view the debriefing.

This set contains 3 tumors, maybe… I'm not a doctor.
Tumor A (slide 19)
Tumor B (slide 19)
Tumor C (slide 23)
Some disclaimer about how the above results aren't always accurate because it's a cheap means of checking and it's always possible something new was found that I didn't know about.
